How Much Does Prostate Surgery Cost in NZ?
"Prostate surgery" is two very different operations with two very different price tags, and most pages on this topic never tell you which one they are describing. Surgery for an enlarged prostate (benign prostatic hyperplasia, or BPH) relieves urinary symptoms by removing part of the gland. Surgery for prostate cancer , a radical prostatectomy , removes the whole gland to treat the disease.

Two operations, two very different costs
The evidence that exists puts enlarged-prostate (TURP) surgery at $8,500 to $11,000 on nib's claims data from December 2022 and August 2023, and self-funded robotic prostatectomy for cancer at $35,000 to $40,000 according to Prostate Matters, the NZ urology charity whose site is maintained by a clinical panel of urologists. There is no official national source for private procedure prices in New Zealand, and urology clinics generally do not publish fees , these are the named, dated figures available.

The nib figures are claims data now several years old , treat them as a floor. As with any private operation, the bill arrives in parts: Mercy Hospital in Dunedin describes separate invoices from the surgeon, the anaesthetist and the hospital, and advises requesting a written cost estimate covering all of them before admission.
Surgery for an enlarged prostate (BPH)
TURP is the standard operation when an enlarged prostate blocks urine flow and medication has stopped helping, and it is a comparatively contained procedure. Health New Zealand's TURP page, last updated 22 July 2026, describes it as recommended when symptoms significantly affect day-to-day living and are not helped by medication: a resectoscope is passed up the urethra and small pieces of prostate are removed, with a catheter in place for a day or two afterwards.
Urology Associates in Christchurch , which, like most NZ urology practices, publishes no prices , describes the surgical options and logistics: TURP takes 30 to 60 minutes depending on prostate size and usually needs one night in hospital, HoLEP uses a laser and suits very large prostates with the most durable long-term results, and Rezūm water-vapour therapy is a newer, less invasive alternative. One night in hospital, a short theatre time and no implant is why BPH surgery sits at the lower end of private surgical costs , the nib claims range of $8,500 to $11,000 reflects that.
Recovery per Health New Zealand: usual activities by 2 to 4 weeks, full healing up to 6 weeks, with heavy lifting and strenuous exercise avoided in that window.
Surgery for prostate cancer
A radical prostatectomy removes the entire prostate gland, and in New Zealand the private version is now overwhelmingly robot-assisted , at a self-funded cost in a different league from BPH surgery. Prostate Matters states that da Vinci surgical robots operate in private hospitals in Auckland, Tauranga, Wellington and Christchurch, that no robotic platforms are currently available in the public system, and that self-funding the robotic operation costs in the order of $35,000 to $40,000. Its clinical panel adds a caution worth repeating: a properly trained, high-volume surgeon matters more to your outcome than the robot does, and open surgery remains available without cost through the public system.

What the public system funds
The public system treats both prostate conditions, with one significant equipment gap. For BPH, publicly funded TURP is available when symptoms are severe enough to qualify , and the queue is the elective queue: the Government's target is for 95 percent of people to wait less than four months for elective treatment, and the official factsheet for the quarter ending March 2026 records performance at 64.5 percent as at December 2025.
For cancer, public treatment is genuinely free but the robotic option is not part of it. Per Prostate Matters, open radical prostatectomy is available through the public system at no cost, while robotic surgery currently is not , which is precisely why the $35,000 to $40,000 self-funded figure exists. Whether the robotic approach is worth that difference is a conversation for your urologist; the charity's own clinical panel emphasises surgeon experience over platform. What health insurance covers for prostate surgery
Prostate Matters states that medical insurers "will almost always cover robotic prostatectomy, but it is important to check your policy" , and the checking is the part that decides real claims. Southern Cross, for example, publishes eligibility criteria for robotic assisted laparoscopic prostatectomy (document last updated March 2015, still published): it reimburses the procedure only when all of its criteria are met, including biopsy-proven prostate cancer, a Gleason score of 7 or below, PSA under 10, BMI under 35, and no visible metastatic spread. A higher-risk cancer or a higher BMI can mean the robotic operation specifically is not reimbursed even though cancer treatment in general is covered.
The other mechanics are the standard ones, with one that matters enormously here:
- Timing against diagnosis. Insurance covers conditions that arise after the policy starts. A prostate cancer already diagnosed , or a PSA result already under investigation , will generally fall within a pre-existing condition exclusion on any new policy. Cover bought after the biopsy will not fund this operation; cover held beforehand is what pays.
- Prior approval. Obtain the written estimate and approval before admission.
- BPH surgery is a routine surgical benefit claim when the condition post-dates the policy , the same pre-existing logic applies to longstanding urinary symptoms already seen by a GP.

ACC has no role in either operation: both BPH and prostate cancer are illness, not accidental injury, so there is no ACC pathway. The funding options are the public system, health insurance held before diagnosis, or self-funding.
